Thank you for giving us the opportunity to be your life-long, trusted dentist! This page was created to provide you with information about how the practice operates, and how we will serve you so that your time and money is well invested in your dental health. You will find information on payment, insurance, our dental emergency policy, and forms to fill out prior to your first visit. 

Appointment

The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

The fees for our services reflect our time, care, skill, judgment, training, and overhead cost factors. To help keep costs down for all of our patients, we have set the following payment requirements:

New patients are required to pay in full at the time of service.

We offer a 5% adjustment for payment in full on complete treatment plans with cash or check.

We accept Visa, Mastercard, American Express, and Discover, as well as cash and check.

We offer CareCredit financing. CareCredit is an interest free loan for up to one year with approval. Please see an office staff member for assistance and more information.

We are currently researching whether having the option to pay bills online is something that our patients want. It is important for us to meet our patients needs and expectations, so we make every effort to stay current with technology and consumer trends.

    

 

Dental Insurance

We accept most dental insurance plans.  We will do our best to assist you in understanding & maximizing your coverage.  However, we are not always able to completely familiarize ourselves with each insurance company's restrictions and requirements.   We no longer accept Medicaid. 

Emergency Policy

Dr. Pinkham only accepts after-hours emergencies for current patients. If you are currently a patient of Associate Dentists, P.C. and you have an after-hours emergency, please call (712) 328-0708 and follow the instructions provided by the answering machine.
